O presente relatório descreve o meu percurso realizado no Estágio, as experiências que vivi, o trabalho que desenvolvi, os conhecimentos que apreendi e acima de tudo o que modifiquei em mim e nos outros nas vertentes da promoção de estilos de vida, diagnóstico, prevenção, reabilitação e reinserção da pessoa com a doença mental. A relação interpessoal enfermeira – utente é base para a manutenção e/ou recuperação da saúde, pelo que todas as intervenções realizadas tiveram como base este pressuposto. No âmbito da intervenção em situação de crise, fiz uma reflexão do percurso profissional pessoal na área de saúde mental e psiquiatria, de forma a tomar consciência das competências especializadas adquiridas. Na vertente de reabilitação e reinserção social participou-se na construção e reformulação de planos individuais de reabilitação e desenvolveu-se um projecto que visou o estímulo cognitivo, contribuindo para a reabilitação psicossocial do doente internado na Unidade de Reabilitação I em Leganés - Madrid. Na área da promoção da saúde, diagnóstico e prevenção de utentes Sem Abrigo, foram identificados problemas e/ou eventuais áreas de intervenção e feita a avaliação do estado de saúde geral dos mesmos. Toda a actuação foi direccionada para a educação para a saúde e realizou-se no “aqui e agora”. Foi também criada uma consulta de enfermagem, sediada no Centro Porta Amiga das Olaias, aberta a toda a população que frequenta o centro. As actividades/intervenções realizadas contribuíram para o autoconhecimento e compreensão das intervenções terapêuticas eficazes na promoção e protecção da saúde mental, na prevenção da doença, no tratamento e na reabilitação objectivando a adequação das respostas da pessoa/doente/família face aos problemas relacionados com a doença mental.
This report describes what I did in my Internship, my experiences, the work developed, the knowledge obtained and most importantly what changed in myself and on the others including in the promotion of healthy lifestyles, prevention, diagnosis, rehabilitation and reintegration of the persons with mental disease. The therapeutic relationship between nurse and patient is the basis for maintaining and/or restoring the health. All interventions realized were based on this concept. As part of the intervention in a crisis situation, it was performed a reflection about personal career in the area of mental health and psychiatry, in order to understand the expertise acquired. In terms of rehabilitation and reintegration into the society, we proceed to a construction and redesign individual rehabilitation plans and it was developed a project which aimed a cognitive stimulation, contributing to psychosocial rehabilitation of patients hospitalized in the Rehabilitation Unit I in Leganés - Madrid. Concerning health promotion, diagnosis and prevention, i worked with homeless patients, performing an assessment of their overall health status and thus indentifying priority areas of intervention. All the action was directed towards education for health and was put in practice on sessions held personally and individually. It was also created a Nursing Consultation Office, in Olaias’s Centro Porta Amiga, open to all population. The activities / interventions performed contributed to self-knowledge, as well as to the understanding of effective therapeutic interventions in promotion and protection of mental health, disease prevention, in treatment and rehabilitation, always aiming at the adequacy of strategies towards the person / patient / family in the resolution of the problems related with mental health.
