Contexto: A Fibrilhação auricular é a arritmia mais frequente na prática clínica, com uma prevalência de 0.4% a 1% na população geral, afectando cerca de 8- 10% da população na oitava década de vida. É uma importante causa de eventos tromboembólicos, aumentando o risco de AVC em 5-6 vezes. A anticoagulação oral é a terapêutica preventiva mais adequada nos indivíduos com FA e alto risco de tromboembolia, mas devido aos diversos inconvenientes e risco hemorrágico associado, os ensaios realizados apontam para uma subutilização na prática médica. Medidas tromboprofiláticas mais rigorosas, mas realistas, são necessárias pois esta já é uma questão considerada por muitos autores como um problema de Saúde Pública. Objectivos: Com este trabalho pretende-se elaborar uma revisão bibliográfica acerca dos diversos aspectos da FA, desde a definição, mecanismos fisiopatológicos e etiologias, passando por factores de risco, diagnóstico e classificação, até à problemática da FA como factor de risco tromboembólico, discutindo terapêuticas antitrombóticas possíveis, os riscos destas, recomendações existentes e as controvérsias implicadas na aplicação na prática clínica destas guidelines. Material e Métodos: Foram pesquisados vários artigos da última década, em motores de busca médicos, por palavras-chave como “atrial fibrillation”, “atrial fibrillation and anticoagulation”, “atrial fibrillation and warfarin”, “atrial fibrillation and stroke”, em várias ocasiões durante o período entre o mês de Novembro de 2007 e Fevereiro de 2008. Conclusões: A FA é uma importante causa de AVC potencialmente modificável, podendo a tromboprofilaxia adequada ter grande impacto na diminuição da incidência das DCV, primeira causa de morte em Portugal. É de realce que estes pacientes tenham participação activa na sua condição e no controlo da sua terapêutica, visando menores complicações associadas e, consequentemente, uma melhor qualidade de vida.
Background: The Atrial Fibrillation is the most commom arrhythmia in clinical practice, with a prevalence of 0.4%-1% in the general population, affecting about 8-10% of the population in the eighth decade of life. It is a major cause of thromboembolic events, increasing the risk of stroke by 5-6 times. The oral anticoagulation is the most appropriate preventive therapy in individuals with AF and high risk of thromboembolism, but due to various problems and bleeding risk associated, the trials point to a underutilization in medical practice. Thromboprophylaxis measures stricter, but realist, are necessary, because this is an issue considered by many authors as a problem of Public Health. Objectives: This paper aims to develop a literature review about different aspects of the AF, since the definition, pathophysiology and etiology, through risk factors, diagnosis and classification, to issue the AF as a risk thromboembolic, discussing possible antithrombotic treatment, these risks, existing recommendations and the controversies involved in the implementation of these guidelines in clinical practice. Methods: It had been searched several articles of the last decade, in scientific browsers by keywords such as "atrial fibrillation", "atrial fibrillation and anticoagulation", "atrial fibrillation and warfarin," and “atrial fibrillation and stroke”, on several occasions during the period between the month of November 2007 and February 2008. Conclusion: The AF is a major cause of stroke potentially modifiable, and may the appropriate thromboprophylaxis have a great impact in reducing the incidence of CVD, the first cause of death in Portugal. It is highlighting that these patients have active participation in its condition and monitoring the therapy, targeting minors related complications, and thus a better quality of life.
