Com este estudo propomo-nos a inventariar e comparar os erros que os técnicos de radiologia cometem durante a execução de radiografias digitais em dois hospitais, situados na região de Lisboa, um com gestão pública e outro privada e, ainda identificar as causas que os originam, dando especial ênfase à repetição de exames no contexto da urgência. A metodologia utilizada incluiu a aplicação de entrevistas guiadas por questionário, o registo de erros e das respectivas causas, a observação directa e a pesquisa documental. Embora a literatura e o registo de erros aponte o mau posicionamento do utente como o erro que mais frequentemente conduz à repetição de exames, os técnicos inquiridos têm uma percepção diferente. A maior parte dos erros encontrados é semelhante em ambos os hospitais e resulta predominantemente da conjugação de factores humanos (sobretudo a distracção) e condições do sistema que propiciam a sua ocorrência (nomeadamente, a sobrecarga de trabalho), o que reflecte o modelo do queijo suíço preconizado por Reason (2000). A partir deste pressuposto, delinearam-se acções preventivas que permitam minimizar o erro humano, à luz dos princípios da Ergonomia e das normas de protecção radiológica, com o objectivo último de melhorar o trabalho de Radiologia em situação de urgência, em prol do bem-estar do profissional de saúde e do doente.
With this study we propose to make an inventory and to compare the errors that the radiographers commit during the execution of digital radiographs in two hospitals, situated in the region of Lisbon, one with public management and other with private management and, also to identify the causes responsible for them, emphasizing the repetition of examinations in the urgency context. The methodology that is used included the application of interviews guided by a questionnaire, the registration of errors and their respective causes, the direct observation and documental research. Although the literature and the registration of errors point out the patient bad positioning as the usual error that most frequently leads to the repetition of the examinations, the questioned radiographers have got a different perception. The majority of the errors which appeared is similar in both hospitals and it proceeds predominantly from the combination of human factors (mainly the distraction) and system conditions that propitiate their occurrence (especially the excess of work), which reflects the Swiss cheese model presented by Reason (2000). From this presupposition, some were delineated preventive actions that can minimize the human error, taking into account the Ergonomics principles and the radiological protection rules, with the last aim to improve the Radiology work in the urgency situation, in favour of health’s professional and patient well-being.
