Resumo: A Política de Saúde Mental Infanto-Juvenil historicamente foi negligenciada, no Brasil e no mundo. Porém, a Convenção Internacional sobre os Direitos da Criança e a promulgação do Estatuto da Criança e do Adolescente (ECA) promoveram uma mudança de paradigma importante que é a compreensão de que crianças e adolescentes são sujeitos de direitos. A Portaria/GM nº 336/02 determinou, dentre as modalidades de atendimento, a criação do Centro de Atenção Psicossocial Infanto-Juvenil (CAPSi ou CAPSij). Esta pesquisa, de caráter qualitativo, objetivou analisar a oferta de cuidados promovida por um Ambulatório de Saúde Mental Infanto-Juvenil em transição para CAPSi, localizado em um município no interior do Estado de São Paulo, a partir do princípio da integralidade preconizado pelo SUS. O processo de transição convocou os trabalhadores para o agir criativo e para a desconstrução de modelos instituídos ¿ desencadeando a sensação de estarem imersos no `olho do furacão¿, como nomearam. Portanto, para a realização deste estudo, a entrada da pesquisadora em campo ocorreu por meio da Pesquisa-Intervenção e a análise dos dados produzidos foi construída à luz da Análise Institucional. Estes referenciais promoveram a possibilidade de encontro entre pesquisadora e sujeitos pesquisados permitindo a identificação dos desafios enfrentados pelo serviço, tais como: produzir cuidados que superem a lógica ambulatorial em um espaço físico restrito, ampliando a clínica de atenção à saúde mental infanto-juvenil mediante a construção coletiva dos projetos terapêuticos; promover acesso ao tratamento e integralidade no cuidado com defasagem de 12 profissionais; investir em apoio matricial diante da ausência de equipes de saúde mental na Atenção Básica, entre outros. Estes referenciais permitiram, igualmente, analisar as dinâmicas relacionais existentes entre os trabalhadores e as instituições que os inscrevem como, por exemplo, a impossibilidade de construção de diálogo aberto em espaços coletivos; produção, em alguns momentos, da lógica de uma clínica privada, mantendo o status quo dos núcleos de atuação. Contudo, apesar dos desafios estampados, esta pesquisa identificou e apontou as potências desta equipe que resistem à alienação do trabalho, como a criação de grupos de acolhimento, possibilitando a compreensão das necessidades de saúde dos usuários; a constituição de miniequipes, otimizando e intensificando os processos de trabalho; o investimento significativo em construções intersetoriais. Por fim, foi possível apontar para a macrogestão estratégias que favoreçam a implantação deste novo modelo de cuidado na atenção à saúde mental infanto-juvenil, como o investimento contínuo em supervisão clínico-institucional, a importância da constituição de equipes de saúde mental na Atenção Básica, o investimento em educação permanente, além do suporte material necessário para a construção do cuidado extramuros
Abstract: The Child and Youth Mental Health Policy has historically been neglected in Brazil and in the world. However, the International Convention on the Rights of the Child and the enactment of the Child and Adolescent Statute (ECA) have promoted an important paradigm shift that is the understanding that children and adolescents are subjects of rights. The Portaria/ GM nº 336/02 determined, among the modalities of attendance, the creation of the Child and Youth Psychosocial Care Center (CAPSi or CAPSij). This qualitative research aimed at analyzing the care offered by a Child and Adolescent Mental Health Ambulatory in transition to CAPSi, located in a municipality in the interior of the State of São Paulo, based on the principle of integrality advocated by SUS. The transition process called on workers to act creatively and to deconstruct established models - triggering the sensation of being immersed in the 'eye of the hurricane', as they have named. Therefore, for the accomplishment of this study, the input of the researcher in the field occurred through the Intervention Research and the analysis of the data produced was constructed in the light of the Institutional Analysis. These references promoted the possibility of a meeting between researcher and researched subjects, allowing the identification of the challenges faced by the service, such as: producing care that surpasses the outpatient logic in a restricted physical space, expanding the clinic of attention to children's mental health through collective construction of therapeutic projects; promote access to treatment and integrality in care with the lag of 12 professionals; to invest in matrix support in the face of the absence of mental health teams in Primary Care, among others. These references have also allowed us to analyze the relational dynamics between workers and their institutions, such as the impossibility of constructing open dialogue in collective spaces; production, at times, of the logic of a private, maintaining the status quo of the actuation nuclei. However, despite the challenges posed, this research identified and pointed out the powers of this team that resist the alienation of work, such as the creation of host groups, enabling the understanding of the health needs of users; the constitution of miniequipes, optimizing and intensifying the work processes; significant investment in intersectoral constructions. Finally, it was possible to point out to macro-management strategies that favor the implementation of this new care model in the attention to children's mental health, such as the continuous investment in clinical-institutional supervision and the importance of the constitution of mental health teams in Primary Care, investment in permanent education, and the material support necessary for the construction of outside walls
