Introduction: Malaria is caused by protozoa, genus Plasmodium, with immunopathogenesis differentiated, related to biological factors to the parasite and the host. According to WHO, there are about 300 to 500 million new cases and one million deaths per year. The pathogenic process initiates by sporozoite inoculation by the vector in the subcutaneous tissues releases merozoites into the circulation forms. Several defense mechanisms are associated with the inflammatory process and the individual's resistance to the pathogen. Objectives: The aim of this study is to evaluate the cellular immune response and determine the frequency of allelic polymorphism of Toll-Like Receptor-4 in patients with malaria, the city of Coari-AM. Material and Methods: For this study, samples were collected from 77 patients infected with P. vivax and 58 samples from uninfected individuals. Immunophenotyping was performed by flow cytometry and levels of cytokines by ELISA and CBA. The determination of the allele frequencies of the polymorphisms of TLR-4 was obtained by PCR-RFLP. Statistical analyzes were performed with the GraphPad Prism 5.0. Results: The results revealed leukopenia and thrombocytopenia, significant increase in the percentage of monocytes, CD4 + T cells and T regulatory cells, and an increase in IL-5, IL-6, IL-8, IL-10 and IFN-y in malaria patients compared with controls. Regarding the Thr399Ile polymorphisms Asp299Gly and TLR-4 was observed in heterozygous frequency of 10% and 5% in malaria patients, respectively. Conclusion: Our data suggest that exist a exacerbation of immune response profile with proand anti-inflammatory cytokines acting without direct influence of polymorphisms in the immunopathogenesis of the disease.

Introdu????o: A mal??ria ?? causada pelo protozo??rio do g??nero Plasmodium, com imunopatog??nese diferenciada, relacionado aos fatores biol??gicos do parasita e do hospedeiro. De acordo com a OMS, ocorrem cerca de 300 a 500 milh??es de novos casos e um milh??o de mortes por ano. O processo patog??nico ?? iniciado com a inocula????o dos esporozo??tos pelo vetor nos tecidos subcut??neos e a libera????o das formas merozo??tas na circula????o. V??rios mecanismos de defesa s??o associados e resultam no processo inflamat??rio e de resist??ncia do indiv??duo ao pat??geno. Objetivos: O presente estudo tem como objetivo avaliar a resposta imune celular e determinar a frequ??ncia al??lica do polimorfismo dos receptores do tipo Toll-4 em indiv??duos com mal??ria, da cidade de Coari-AM. Material e M??todos: Para este estudo foram coletadas amostras de 77 pacientes infectados com P. vivax e 58 amostras de indiv??duos n??o infectados. A imunofenotipagem foi realizada pela t??cnica de citometria de fluxo e as dosagens de citocinas por ELISA e CBA. A determina????o das frequ??ncias al??licas dos polimorfismos do TLR-4 foi obtida por PCR-RFLP. As an??lises estat??sticas foram realizadas com o programa GraphPad Prism 5.0. Resultados: Os resultados revelaram que os pacientes apresentaram quadros de leucopenia e trombocitopenia, aumento significativo no percentual de mon??citos, linf??citos T CD4+ e c??lulas T regulat??rias, al??m do aumento nas citocinas IL-5, IL-6, IL-8, IL-10 e IFN-y nos pacientes com mal??ria em compara????o com os controles. Em rela????o aos polimorfismos Asp299Gly e Thr399Ile do TLR-4, foi observada frequ??ncia em heterozigose de 10% e 5% nos pacientes com mal??ria, respectivamente. Conclus??o: Em conclus??o, nossos dados sugerem processo de exacerba????o da resposta imune, com perfil pr??e anti-inflamat??rio atuante, sem influ??ncia direta dos polimorfismos na imunopatog??nese da doen??a.
