The involvement of the skin in sarcoidosis is present in about 5 to 35% of cases, and it may be the initial manifestation of the disease.The authors describe the clinical case of a 34 year old female patient, hospitalized with high fever, a 20kg weight loss and an extensive paniculitis of the inferior quadrants of the abdomen, with about 2 months of evolution.The biopsy revealed a granulomatous lobular paniculitis without vasculitis and without caseous necrosis of sarcoidosis type.All the other diseases that can cause a lobular paniculitis without vasculitis were excluded.The patient evolved favourably with the institution of oral corticotherapy and the follow-up at 3 years excluded the involvement of other organs.
Na sarcoidose, o envolvimento cutâneo está descrito em 5 a 35% dos casos, podendo constituir a manifestação inicial de doença.Os autores descrevem o caso clínico de uma doente de 34 anos, internada por febre elevada, emagrecimento de 20kgs e paniculite extensa dos quadrantes inferiores do abdómen, com cerca de dois meses de evolução.A biopsia cutânea revelou paniculite lobular granulomatosa sem vasculite e sem necrose caseosa de tipo sarcoide. Excluiram-se todas as outras doenças que podem cursar com paniculite lobular sem vasculite.A doente evoluiu favoravelmente com corticoterapia oral durante 6 meses, e o seguimento clínico não revelou envolvimento de outros órgãos ao fim de três anos.
