Pain is an entity that has accompanied human beings since the beginning of their existence. Neuropathic pain, induced by alterations in the somatosensory system, has its own characteristics and treatment. Gabapentinoids are antiepileptic drugs that show marked similarities to gamma-aminobutyric acid (GABA), one of the main inhibitory neurotransmitters in the central nervous system (CNS). Since the discovery of their applicability in the treatment of neuropathic pain, their use in this field has had a steady increase. The most recent scientific evidence on gabapentinoids, however, seems to point to less than desirable efficacy in their use in the treatment of neuropathic pain, with new national guidelines emerging with the switch from pregabalin to second-line treatment in this area. It is therefore important to develop new scientific studies that will shed light on the real role of gabapentinoids in the treatment of neuropathic pain and the discovery of new courses of action and alternative therapies that are effective in this area of pain. A dor constitui uma entidade que acompanha o ser humano desde o início da sua existência. A dor neuropática, induzida por alterações a nível do sistema somatossensorial apresenta caraterísticas e tratamento próprios. Os gabapentinóides são fármacos antiepiléticos que apresentam semelhanças marcadas com o ácido gama-aminobutírico (GABA), um dos principais neurotransmissores inibitórios do sistema nervoso central (SNC). Desde a descoberta da sua aplicabilidade no tratamento da dor neuropática que o seu uso neste campo tem vindo a aumentar paulatinamente. As evidências científicas mais recentes sobre os gabapentinóides, no entanto, parecem apontar para uma eficácia aquém do desejável, no seu uso no tratamento da dor neuropática, com novas guidelines nacionais a surgirem com a passagem da pregabalina para tratamento de 2ª linha nesta área. É, assim, importante desenvolver novos estudos científicos que permitam esclarecer o real papel dos gabapentinóides no tratamento da dor neuropática e a descoberta de novas vias de atuação e terapêuticas alternativas, eficazes nesta área da dor.
