The patients evacuation from portuguese speaking countries in Africa to Portugal - evaluative study

In the scenario of international cooperation, in a perspective of support to the development of health in Portuguese speaking African Countries (PALOP), Portugal (PT) has existing cooperation agreements signed between 1977 and 1992, allowing the evacuation of patients (EVD) from PALOPs to Portuguese public hospitals in search of solutions to health problems not resoluble in their countries of origin. The procedures involved in their application were clarified by Normative Circular 04/ DCI of April 16, 2004, still in force. The EVD is regulated and coordinated by the General Board of Health (DGS), and funded by the Ministry of Health. Patients arrive in PT under the agreements but also out of these. They mainly come from Guinea- Bissau (GB) and Cape Verde (CV). Apparently the evaluation of the efficiency and effectiveness of this service wasn’t properly described. Focusing on new approaches to support the development and ambitioning to contribute to the health development in the PALOPs, one will seek to evaluate the scenario of EVD for therapeutic purposes for PT. The research strategy will undergo perform an evaluation research, in particular an analysis of intervention and implementation, converging on a strategic analysis using a multiple case study. Will analyse CV, GB and PT, starting from data collection using three different techniques, which will be i) document analysis, ii) semi-structured interviews focused on the problem addressed to leaders and experts in the field, using a non-probability intentional sampling iii) semi-structured interviews focused on the problem addressed to patients evacuated and to professionals who receive them in PT, using a snowball sampling. Crossing the data obtained from existing documentation, from the life experiences of those who are evacuated and from the vision of those to whom this topic is not indifferent, one dares to expect suggesting recommendations to apply to this field in order to promote support for the development of health in the PALOPs and reinforce health policy of these states.

No cenário da Cooperação Internacional, numa perspetiva de apoio ao desenvolvimento sanitário dos Países Africanos de Língua Oficial Portuguesa (PALOP), existem em Portugal (PT) Acordos de Cooperação, assinados entre 1977 e 1992, permitindo a evacuação de doentes (EvD) oriundos dos PALOP para hospitais públicos portugueses, em busca de solução para problemas de saúde não resolvíveis nos seus países de origem. Os procedimentos inerentes à sua aplicação foram clarificados pela Circular Normativa nº04/DCI de 16 de Abril de 2004, em vigor. A EvD é regulada e coordenada pela Direção Geral de Saúde (DGS), sendo financiada pelo Ministério da Saúde. Os doentes chegam a PT ao abrigo dos acordos mas também à margem destes. Provem sobretudo da Guiné Bissau (GB) e de Cabo Verde (CV). Aparentemente não se encontra devidamente descrita a avaliação da eficácia e da eficiência deste serviço. Centrando o pensamento nas novas abordagens de apoio ao desenvolvimento e ambicionando contribuir para o desenvolvimento sanitário dos PALOP, propõe-se a realização de um estudo que procurará avaliar o cenário da EvD com fins terapêuticos para PT. A estratégia de investigação passará pela realização de uma pesquisa avaliativa, concretamente uma análise de intervenção e de implementação, convergindo para uma análise estratégica, recorrendo a um estudo de casos múltiplos. Analisar-se-á CV, GB e PT partindo da recolha de dados com recurso a três técnicas distintas, sendo elas i) análise documental, ii) entrevistas semiestruturadas centradas no problema dirigidas a responsáveis e peritos na matéria, recorrendo a uma amostragem não probabilística intencional iii) entrevistas semiestruturadas centradas no problema dirigidas a doentes evacuados e a profissionais que em PT os recebem, recorrendo a uma amostragem em bola de neve. Cruzando os dados obtidos da documentação existente, das histórias de vida de quem é evacuado e da visão daqueles a quem esta temática não deixa indiferente, ousa-se esperar conseguir sugerir recomendações a aplicar nesta matéria no sentido de promover o apoio ao desenvolvimento sanitário dos PALOP e reforçar a política de saúde destes Estados.
