The authors assess the contributions of Portugal to the development of health systems in the Portuguese African empire until the early 20th century. The marine medicine, the support of health facilities in mainland Africa and the interventions of the Misericórdias are touched upon. Only after the French Revolution, the independence of Brazil and the liberal war in Portugal, under the pressure of the English Ultimatum and following the Berlin Treaty, the Portuguese Government undertook effective occupation of its African territories, developing health infrastructures tailored for the needs of the overseas’ colonies. The solutions found over the centuries reflect the scientific knowledge of the moment, the socially appreciated values in the European societies and the interest in health as a matter of public policy.

Analisa-se o contributo dos portugueses para o desenvolvimento dos sistemas de assistência sanitária no império português em África até ao início do século XX. Consideram-se a medicina a bordo, as instituições de apoio em terra, em África e a intervenção das Misericórdias. Só depois das convulsões resultantes da Revolução Francesa, que levaram à independência do Brasil e ao Tratado de Berlim e Ultimato inglês, é que o Estado português apostou numa ocupação efetiva dos seus territórios africanos, desenvolvendo estruturas de saúde pensadas para as colónias ultramarinas. As respostas encontradas ao longo dos tempos refletem os conhecimentos científicos de cada época, os valores prezados nas sociedades europeias e a visão do papel do Estado no setor da saúde.
