Equity and governance: political analysis of research in the social determinants of health in Brazil

The strengthening of the health research system requires monitoring of funding policies. This study analyses the funding policy structures, the interinstitutional governmental collaboration and the repercussion of research capacity in social determinants of health, under a perspective of equity, in Brazil. This is an evaluative research based on the principle of authentic partnership within Health Global Research applied to funding policies. Data from the Ministry of Health former calls and funding research, from 2004 to 2014, were identified in institutional promotion websites and digital platform. One hundred fifty-five researchers were financed in the amount of R$ 25.2 million. The Ministry of Health invested significantly with the Health and Research State Institutions. The majority of calls did not clearly stated this theme and among the national calls that prioritized it influenced the financing. The capacity of institutional research is highly concentrated in three institutions headquartered in the Southeast of Brazil. This panorama indicates challenges concerning the development of effective governance mechanisms: interinstitutional management and financial collaboration, research priorities definition and enhance the growth of research groups on socioeconomic, gender, ethnic and regional inequalities in the country. 

O fortalecimento do sistema de pesquisa em saúde requer o monitoramento das políticas de financiamento. Este estudo analisa a estrutura da política de financiamento, a colaboração interinstitucional governamental e as repercussões na capacidade de pesquisa em determinantes sociais da saúde, na perspetiva da equidade, no Brasil. Trata-se de pesquisa avaliativa, baseada no princípio da parceria autêntica da Pesquisa Global em Saúde aplicado a políticas de financiamento. Dados dos editais e do financiamento das pesquisas do Ministério da Saúde, no período de 2004 a 2014 foram identificados em sites institucionais de fomento e plataforma digital. Foram financiadas 155 pesquisas, no valor de R$ 25,2 milhões. O Ministério da Saúde cooperou de forma relevante com as instituições estaduais de saúde e pesquisa. A maioria dos editais não explicitou esse tema e entre os editais nacionais que o priorizaram influenciaram no financiamento. A capacidade de pesquisa institucional é altamente concentrada em três instituições sediadas na região Sudeste. Esse panorama indica desafios associados com o desenvolvimento de mecanismos efetivos de governança: gestão interinstitucional e de colaboração financeira, definição de prioridades de pesquisa; e formação de grupos de pesquisa em desigualdades socioeconómicas, de género, étnicas e regionais no país.
