Abstract Primary aldosteronism (PA) is a difficult and an easily overlook diagnosis in pregnancy and postpartum period, resulting in an underdiagnosed condition. We report a case of PA diagnosed in the postpartum period. A 31-year-old woman was presented with severe resistant hypertension at emergency room at 35 weeks pregnancy. After delivery, she maintained severe resistant hypertension and hypokalemia which led to a diagnosis of PA due to an adrenal adenoma. Secondary causes of hypertension should be considered in cases of resistant hypertension in order to prevent adverse maternal outcomes.
