Avaliação acústico-perceptiva e stress em mulheres com patologia laríngea

Objectivo / Tema: O objectivo deste estudo foi a recolha de dados acústicos, perceptivos e psicológicos de sujeitos do género feminino que possuíssem patologia laríngea com a finalidade de realizar uma avaliação mais abrangente e contribuir para a precisão do diagnóstico clínico em Terapia da Fala bem como demonstrar a importância da avaliação acústica como um complemento à avaliação vocal perceptiva, facilitando a compreensão da patologia a partir de dados objectivos. Metodo: Mediante o diagnóstico ORL foram seleccionados os sujeitos do género feminino com a patologia laríngea de nódulos vocais e na primeira consulta de Terapia da Fala foram preenchidas as fichas de caracterização dos sujeitos (pela TF), foram captados os sinais acústicos (gravação vocal das vogais /a/, /i/ e /u/ durante a leitura de uma frase de apoio e conversação) e por fim foi preenchida a Escala de Auto-Avaliação da ansiedade de Zung (pelo doente). Resultados: Como principais resultados verificou-se que os parâmetros acústicos mais afectados foram a F0 e o Jitter PPQ5 em conversação e o Shimmer APQ3 e HNR em leitura e conversação. Os parâmetros perceptivos mais afectados foram o grau da da disfonia (G), a rouquidão (R) e a tensão (S). Na avaliação psicológica, ocorreram maior número de indivíduos com grau de ansiedade patológica. A ansiedade revelou um efeito significativo nos valores de F0, Jitter PPQ5, Shimmer APQ3 e HNR. Observou-se uma correlação negativa significativa entre o G e a F0 em leitura e o G e o HNR em conversação e leitura. Ocorreu uma correlação positiva significativa entre o R e o Jitter PPQ5 , o R e o Shimmer APQ3 e o G e o Jitter PPQ5 para a leitura e conversação. Discussão: Os resultados obtidos neste trabalho vêm ao encontro de muitos estudos (para outras línguas) consultados na revisão bibliográfica realizada, tendo-se verificado uma relação entre a avaliação psicológica e acústica e entre a avaliação acústica e perceptiva. ABSTRACT: Objective / Theme: The aim of this study was to gather the acustic, perceptive and psychological dada of female subjects who had laryngeal pathology. This study has the final purpose of attaining a broader evaluation and of contributing to the clinical diagnoses’ precision in Speech Therapy as well as to demonstrate the importance of the acustic evaluation as a complement to the perceptive vocal evaluation, making easier to understand the pathology from objective dada. Method: By means of the ORL diagnoses, the female subjects with laryngeal pathology of the vocal nodules were selected, and at the first consultation of Speech Therapy all subject’s caracterization notes were filled (by the ST), the acustic signals (vocal recordings of the vocals /a/, /i/ and /u/ during the reading of a backup line and conversation) were captured, and finally the Zung’s Anxiety Auto-Evaluation Scale was filled (by the patient). Results: As major results it was established that the most afected acustical parameters were the F0 and the Jitter PPQ5, in conversation, and the Shimmer APQ3 and the HNR, in reading and conversation. The most afected perceptive parameters were the degree of dysphonia (G), roughness (R) and tension (S). At the psychological evaluation there were more individuals with a degree of pathological anxiety. This anxiety showed a significant effect on the values of F0, Jitter PPQ5, Shimmer APQ3 and HNR. It was observed a significant negative correlation between G and F0 regarding reading, as well as between G and HNR regarding conversation and reading. It occured a significant positive correlation between R and the Jitter PPQ5, the R and the Shimmer APQ3 and the G and the Jitter PPQ5 regarding reading and conversation. Discussion: The results of this study are similar too many studies (for other languages) found in the literature review conducted and it has been verified a close relation between the psychological and the acoustic evaluation and between the acoustic and perceptual assessment.
