INTRODUÇÃO: O ceratocone é uma doença ocular, não infecciosa, na qual a córnea enfraquece devido a anomalias estruturais e da sua composição. Isso enfraquece a córnea não permitindo que a mesma preserve a sua forma normal contra a pressão intra-ocular no olho e, portanto, há protusão, levando a uma forma cónica e visão distorcida subsequente. Mudanças na estrutura e composição muitas vezes manifestam-se como uma alteração de forma (ou geometria), bem como nas propriedades mecânicas e ópticas. Esta afecção é a distrofia mais comum da córnea, acometendo 50 a 230 pessoas por 100.000 habitantes, e normalmente inicia-se como miopia ou astigmatismo ou ambos, podendo evoluir rapidamente, ou então, levar muitos anos para desenvolver-se. Costuma surgir na adolescência, por volta dos 15 anos de idade e dificilmente desenvolve-se após os 35 anos. Normalmente ocorre assimetricamente, sendo que em 90% dos casos, acomete ambos os olhos; todavia, o diagnóstico da doença no segundo olho ocorre aproximadamente 5 anos após o diagnóstico no primeiro olho. Não parece ter preferência pelo sexo ou etnia. Este estudo pretende correlacionar os diferentes tipos de ceratocone com as diversas abordagens terapêuticas disponíveis, nomeadamente o tratamento médico e/ou cirúrgico. Passa por fazer uma análise retrospectiva de dois grupos de doentes em que se compara a acuidade visual antes e após dois tipos de tratamento cirúrgico – INTAC e anéis Ferrara. OBJECTIVOS: 1)Identificar os diferentes tipos de ceratocone. 2)Caracterizar as diversas abordagens terapêuticas. 3)Comparar a acuidade visual inicial dos doentes com a resultante após o tratamento cirúrgico – INTACs e aneis Ferrara. 4)Analisar se há melhoria da acuidade visual com correcção pós-operatória. 5)Verificar se há alguma diferença, estatisticamente significativa, da acuidade visual com correcção, no tratamento do ceratocone, utilizando anéis Ferrara ou INTACs. MATERIAIS E MÉTODOS: Este estudo avaliou 62 olhos de 36 pacientes (22 homens, 14 mulheres) com uma média de idades de 31,42 anos ±11,71(DP) (variação 18-75 anos) com o diagnóstico clínico de ceratocone sendo que em 32 casos (51,6%) recorreu-se a anéis Ferrara, e em 30 casos (48,4%) a INTACs. RESULTADOS: Verificou-se que houve uma melhoria da acuidade visual com correcção em média de 5,87/10 ±1,67 (DP) (variação 2/10-9/10). Apesar de a melhoria da a. v. c/ c. média no grupo INTAC (6,03/10) ser superior à do grupo Ferrara (5,72/10), esta diferença não é estatisticamente significativa (P=0,464). No grupo Ferrara em 28,1% dos casos houve melhoria de 5/10 de visão e no grupo INTAC em 25% dos casos houve melhoria de 5/10 de visão. De notar que houve melhoria da acuidade visual com correcção, estatisticamente significativa (p=0,000), em todos os olhos estudados após intervenção cirúrgica. CONCLUSÃO: Não existem diferenças, estatisticamente significativas, na melhoria da acuidade visual resultante do tratamento cirúrgico com anéis Ferrara ou INTACs.
INTRODUCTION: Keratoconus is a non infectious eye disease, that weakens the cornea due to structural abnormalities and their composition. This weakening of the cornea doesn’t allow it to maintain its normal shape against the intraocular pressure in the eye and therefore, protrudes, leading to a conical shape and subsequent blurred vision. Changes in structure and composition often manifest as an alteration in shape (or geometry) as well as in the mechanical and optical properties. This condition is the most common corneal dystrophy, affecting 50 to 230 persons per 100,000 inhabitants, and usually begins as myopia, astigmatism or both, and may progress rapidly, or else take several years to develop. Usually arises in adolescence, around 15 years old and hardly develops after the age of 35. Generally occurs asymmetrically, and in 90% of cases, affects both eyes, however, the diagnosis of the disease in the second eye occurs approximately 5 years after the diagnosis in the first eye. It doesn’t seem to have a preference for gender or ethnicity. This study aims to correlate the different types of keratoconus with the different therapeutic approaches available, including medical and surgical treatment. Starts by making a retrospective analysis of two groups of patients comparing the visual acuity before and after two types of surgery - INTACs and Ferrara rings. OBJECTIVES: 1)Identify the different types of keratoconus. 2)Characterize the different therapeutic approaches. 3)Compare the initial visual acuity of patients with results after surgical treatment - Ferrara and INTACs rings. 4)Consider whether there is an improvement in visual acuity with postoperatively correction. 5)Check for any difference, statistically significant, on visual acuity after correction of keratoconus using INTACs or Ferrara rings. MATERIALS AND METHODS: This study evaluated 62 eyes of 36 patients (22 men, 14 women) with a mean age of 11.71 ± 31.42 years (SD) (range 18-75 years) with clinically diagnoses of keratoconus; in 32 cases (51.6%) resorted to Ferrara rings, and in 30 cases (48.4%) to INTACS. RESULTS: We found that there was an improvement in visual acuity with correction on average of 5.87 / 10 ± 1.67 (SD) (range 2/10-9/10). Despite improvements in the a. v. c / c. on average of INTAC group (6.03 / 10) is higher than the Ferrara group (5.72 / 10), this difference isn’t statistically significant (P = 0.464). In 28.1% of cases in the Ferrara group there was improvement in 5/10 vision and in 25% of cases in the INTAC group there was improvement in 5/10 vision. Please note, that there was improvement in visual acuity with correction, statistically significant (p = 0.000), in all eyes after surgery. CONCLUSION: There are no differences, statistically significant, in improvement of visual acuity resulting from surgical treatment with INTACs or Ferrara rings.
