A evidência científica tem demonstrado claramente que a depressão é um dos problemas mais graves de saúde pública, sendo o distúrbio psiquiátrico mais comum nos idosos. Traduz-se inevitavelmente numa sobrecarga para as sociedades, prevendo-se o seu incremento no futuro. Acreditando que é possível contribuir para a promoção da saúde mental no idoso a nível da comunidade, como enfermeiro a frequentar a Especialidade de Enfermagem de Saúde Mental e Psiquiátrica direccionei o estágio para esta área. O objectivo final foi demonstrar até que ponto as intervenções de Enfermagem Especializada junto do idoso com depressão, podem converter-se em ganhos em saúde. Numa primeira fase elaborei um Diagnóstico de Situação, onde caracterizo a população, composta por 46 utentes inscritos no Centro de Dia. Esta inclui, a identificação da pessoa; os antecedentes pessoais; uma avaliação breve do estado mental, através da aplicação do Mini-Mental State Examination; uma avaliação do estado depressivo, pela aplicação da Escala de Depressão Geriátrica; e ainda um último ponto designado “generalidades” que permite aprofundar o conhecimento da pessoa. Numa segunda fase, realizei intervenções de enfermagem de saúde mental, caracterizadas essencialmente por entrevistas de ajuda, de acordo com Phaneuf, dirigidas a 6 utentes identificados com depressão ligeira e sem defeito cognitivo. Optei pelo estudo exploratório, descritivo, seguindo a metodologia do estudo de caso, para trabalhar com cada um dos 6 utentes anteriormente referidos. A recolha de dados baseou-se inicialmente na aplicação da metodologia do formulário e posteriormente, nas orientações preconizadas pelo modelo co-activo para o estudo de caso. Tendo em conta os objectivos e tipo de estudo, a estratégia adoptada para o tratamento e análise dos dados foi qualitativa e quantitativa. Os ganhos obtidos em saúde mental, evidenciaram-se pela redução da pontuação nas Escalas de Depressão Geriátrica.
The scientific evidence has clearly shown that depression is one of the most serious problems in public health, being the most common psychiatric disturb disorder in the elderly. Inevitably, this is becoming an increasing burden on society. Believing that it is possible to contribute for the promotion of the mental health in the elderly at a community level, I, as nurse attending a specialty in mental health and psychiatric, focused the two clinical tests for this area. The main goal was to demonstrate until which point the specialized nursing interventions near the elderly with depression, may convert in health benefits. In a first stage, a diagnosis situation was elaborated in order to characterize a population of forty six users registered in the Oeiras Day Centre. This diagnosis includes the personal identification; medical background of the person; a short evaluation of the mental state using for that the Mini-Mental State Examination; an evaluation of the depressive state by the Geriatric Depression Scale and as a last point a section named “generalities”, which permit to deepen the knowledge about the person. In a second stage, nursing interventions according with Phaneuf for mental health were held. These were mainly characterized by help interviews aimed to the six users that were identified with slight depression and without cognitive defect. The study is exploratory-descriptive and a case study methodology to work with each of the six users mentioned above was followed. Initially, the data collection is based in a form methodology and in a later stage the guidelines recommended by the co-active model for the case study. Considering the goals and the type of study in cause, the strategy adopted for the processing and data analysis was qualitative and quantitative. In terms of the results, it was observed, by comparing the scores of Geriatric Depression Scale application, that all the followed users had really acquired benefits in mental health.
