We report a case of "spontaneous" bladder rupture in a child with type IV Ehlers-Danlos Syndrome phenotype. Pneumoperitoneum is rarely seen with bladder rupture. In blunt abdominal trauma this sign is strongly suggestive of gastric or bowel perforation.
