Metformin, one of the most widely prescribed oral hypoglycemic agents, has recently increased attencion because of its potencial antitumorogenic effects that are thought to be independent of its hypoglycemic effects. Several observacional and biological studies revealed a significant association between metformin and a dose-dependent reduction in cancer incidence, and with a better response to chemotherapy. The mechanisms by which metformin exerts these effects are mediated through activation of AMP-activated protein kinase (AMPK), inhibition of the mammalian target of rapamycin (mTOR) pathway, and inhibition of insulin like growth factors (IGFs), and many others. This review highlights basic aspects of the molecular biology of metformin and summarizes new advances in the anti-neoplastic activities of metformin as well as intriguing results from recent clinical studies.

A metformina, um dos agentes hipoglicemiantes orais mais amplamente prescrito, recebeu recentemente maior atenção devido aos seus potenciais efeitos antitumorogénicos, os quais se pensam ser independentes dos seus efeitos hipoglicémicos. Diversos estudos observacionais e biológicos revelaram uma associação significativa entre a metformina e a redução da incidência de cancro, de uma forma dose-dependente, bem como uma melhor resposta à quimioterapia. Os mecanismos através dos quais a metformina exerce estes efeitos são mediados pela ativação da proteína cinase AMPK, inibição da via do mammalian target of rapamycin (mTOR), inibição dos fatores de crescimento insulina-like (IGFs), e muitos outros. Esta revisão destaca aspetos básicos da biologia molecular da metformina e sumariza novos avanços na atividade antineoplásica da metformina, integrando resultados de estudos clínicos recentes.
