Description

Introduction: Temporomandibular disorders (TMD), including bruxism, affect approximately 10-30% of the population and lead to pain, tooth wear and occlusal overload, negatively impacting quality of life and the longevity of oral rehabilitation. Botulinum toxin type A (BoNT-A) has emerged as an adjunctive therapeutic approach due to modulate masticatory muscle activity. Objective: To evaluate the clinical efficacy and safety of botulinum toxin type A in the treatment of bruxism, with particular emphasis on the modulation of electromyographic activity, bite force and associated myofascial pain. Material and Methods: A systematic integrative review of articles published over the last 10 years (2016–2026) in the PubMed database, following the PICO strategy and PRISMA guidelines, along with a manual search in the secondary source ResearchGate. A total of 15 articles were included. Results: The administration of BoNT-A into the masticatory muscles has confirmed significant reductions in myofascial pain, EMG activity, and bite force. The therapeutic effect is typically visible between the first and 4th week. Therapeutic efficacy is maintained for 3 to 4 months. Long-term studies indicate that the therapeutic effect persists for up to 6 months to 1 year. Discussion: Applications between 30U and 100U total of BoNT-A in the MM isolated and/or associated with 10U-15U total in the MT and/or MPTL have proven to be effective and safe. Doses ≥ 100U total showed a higher risk of possible adverse effects, which were mild and reversible. The application points vary between 2-3 points for the MM and 1-2 for the temporal and pterygoid muscles. There is a marked heterogeneity in the protocols and clinical practices adopted Conclusion: BoNT-A may, therefore, be an effective, safe, and clinically relevant therapeutic approach when used in combination with traditional therapies, improving patients' quality of life, tooth wear, and the longevity of oral rehabilitations. The increasing use of BoNT-A highlights the need for future studies aimed at standardizing protocols according to disease severity and specific patient characteristics
