Description

Breastfeeding is recognised as one of the most effective interventions for promoting maternal and child health, with well-documented short- and long term benefits. However, significant disparities persist in initiation and maintenance rates, particularly among migrant populations, where cultural, linguistic, and structural factors often constrain access to appropriate care. In Portugal, the integration of culturally sensitive and interdisciplinary approaches within primary healthcare remains limited, and the role of the Speech and Language Therapist (SLT) in breastfeeding promotion is still underexplored, despite their specific expertise in orofacial motricity, swallowing, and early feeding. In this context, the present thesis aimed to explore and evaluate the contribution of SLTs to the promotion and support of breastfeeding among migrant women followed in primary healthcare. Based on the premise that breastfeeding is both a biological and relational process, the research integrated SLTs’ clinical expertise in orofacial function and swallowing with culturally responsive and mother–infant-centred approaches. Five interrelated studies were conducted. The first, a systematic review, identified a scarcity of high-quality, culturally adapted interventions targeting migrant women, confirming the absence of SLTs within breastfeeding support teams. The second study explored the breastfeeding knowledge, attitudes, and practices of 20 migrant women living in Portugal, revealing information gaps, language barriers, and discrepancies between intentions and practices. Based on these findings, a culturally adapted, SLT-led structured intervention was developed and tested in a pilot study with 18 migrant women (third study), which demonstrated high feasibility, acceptability, and positive trends in exclusive breastfeeding, maternal self-efficacy, and postpartum quality of life. The fourth study, a randomised controlled trial with 74 migrant women, confirmed the effectiveness of this intervention, showing significantly higher proportions of exclusive breastfeeding at 1, 3, and 6 months in the intervention group (IG) compared with the control group (CG), higher maternal self-efficacy, better postpartum quality of life, and more efficient infant oromotor function. Finally, the fifth study analysed the perceptions of 53 nurses and physicians regarding the SLT’s role in breastfeeding, recognizing their potential contribution but also the need to integrate more SLTs in primary healthcare to increase referral pathways and interdisciplinary collaboration. Taken together, the findings of this thesis demonstrate that integrating SLTs into breastfeeding support programmes is feasible, effective, and clinically relevant, contributing to a more comprehensive and equitable approach to maternal and child health. This work represents both a conceptual and practical advance in speech-language therapy, positioning SLTs as key professionals in the prevention and promotion of health from the very beginning of life.
