We describe a clinical case of a patient with osteosarcoma, who underwent resection and reconstruction with an expandable prosthesis, mechanical failure of the lengthening device and sudden dysmetria. Girl, 8 years old, with distal metadiaphyseal osteosarcoma of the left femur. She underwent a 14cm resection and reconstruction with an expandable prosthesis. Mechanical lengthening without complications, totaling 4.4 cm. Clinical dysmetria of 2 cm at maturity. At 16, she suffered a fall which damaged the mechanical system of the prosthesis and caused the lengthening to collapse. She opted for revision with an electromagnetic lengthening prosthesis (4.6 cm over 2 months). At the age of 18, it was replaced with a definitive modular prosthesis. At 6 months post‑surgery, he had no pain and radiological dysmetria of 2 cm, with no need for compensation. We describe an unusual case in which we used an electromagnetic lengthening prosthesis to solve a complication of a mechanical expandable prosthesis.
Descrevemos o caso clínico de uma doente com osteossarcoma, submetida a ressecção alargada e reconstrução com prótese expansível, na qual se verificou falência mecânica do dispositivo de alongamento e dismetria súbita. Menina, 8 anos, com osteossarcoma metadiafisário distal do fémur esquerdo. Submetida a resseção de 14 cm e reconstrução com prótese expansível. Alongamentos mecânicos sucessivos sem complicações, perfazendo 4,4 cm. Dismetria clínica de 2 cm à maturidade. Aos 16 anos sofreu queda que danificou o sistema mecânico da prótese e causou colapso do alongamento. Optou‑se por revisão com prótese de alongamento electromagnético (4,6 cm durante 2 meses). Aos 18 anos, substituiu‑se por prótese modular definitiva. Aos 6 meses pós‑operatório encontra‑se sem queixas álgicas, com dismetria radiológica 2 cm, sem necessidade compensação. Descrevemos um caso invulgar, em que utilizámos uma prótese de alongamento eletromagnético para resolver uma complicação associada a uma prótese expansível mecânica.
