The new International Classification of Orofacial Pain (ICOP) (1st Edition), a joint venture of the International Headache Society, American Academy of Orofacial Pain (AAOP), Orofacial and Head Pain Special Interest Group (OFHP SIG) of the International Association for the Study of Pain (IASP) and International Network for Orofacial Pain and Related Disorders Methodology (INfORM), lists all causes of orofacial pain defining strict diagnostic criteria, specifying clinical features of all the clinical entities that can produce orofacial pain. In the ICOP, the hierarchical design and classification style of the International Classification of Headache Disorders (ICHD-3) was adopted. Overall, ICOP is also aligned to the International Classification of Diseases 11th Revision (ICD-11)/International Association for the Study of Pain (IASP) criteria for orofacial pains. The aim was therefore to create a tool that will enhance the research and clinical management of orofacial pain. Secondary causes and primary causes of pain in this anatomical region are considered. Orofacial pain was divided in 6 chapters: 1. Orofacial pain attributed to disorders of dentoalveolar and anatomically related structures, 2. Myofascial orofacial pain, 3. Temporomandibular joint (TMJ) pain, 4. Orofacial pain attributed to lesion or disease of the cranial nerves, 5. Orofacial pains resembling presentations of primary headaches and 6. Idiopathic orofacial pain. There is also a final chapter on 7. Psychosocial assessment of patients with orofacial pain. We present the Portuguese translation of the ICOP, a collaborative work between the Portuguese Headache Society, the Portuguese Association for the Study of Pain and the Portuguese Society for Temporomandibular disfunction, Orofacial Pain and Sleep. A nova Classificação Internacional de Dor Orofacial (primeira edição), elaborada em conjunto por várias sociedades científicas (International Headache Society, American Academy of Orofacial Pain (AAOP), Orofacial and Head Pain Special Interest Group (OFHP SIG) of the In-ternational Association for the Study of Pain (IASP) e International Network for Orofacial Pain and Related Disorders Methodology (INfORM)), lista as entidades que podem provocar dor nesta região anatómica definindo critérios de diagnóstico rigorosos, especificando as características clínicas e requisitos necessários para a realização de um diagnóstico preciso. Inspirada no modo de organização da Classificação Internacional de Cefaleias (ICHD-III), no geral, está alinhada com os critérios da Classificação Internacional de Doenças, 11ª revisão (ICD-11, International Classification of Diseases 11th Revision) e com os critérios da Associação Internacional para o Estudo da Dor (IASP, International Association for the Study of Pain) no que respeita às dores orofaciais e cefaleias. Estão incluídas uma longa lista de dores secundárias e as suas causas, mas também as entidades idiopáticas que se apresentam com dor orofacial. Esta classificação abrangente e detalhada, com terminologia precisa facilita a comunicação e o entendimento entre as várias especialidades médicas implicadas no diagnóstico e tratamento das várias patologias. Os diferentes tipos de dor orofacial foram divididos em 6 capítulos: 1. Dor orofacial atribuída a perturbações dentoalveolares e estruturas anatómicas associadas, 2. Dor orofacial miofascial, 3. Dor da articulação temporomandibular (ATM), 4. Dor orofacial atribuída a lesão ou doença dos nervos cranianos, 5. Dores orofaciais com apresentação semelhante às cefaleias primárias e 6. Dor Orofacial Idiopática. Existe ainda um capítulo final sobre 7. Avaliação psicossocial de doentes com dor orofacial. Apresentamos a tradução para português desta classificação, iniciativa conjunta da SPC (Sociedade Portuguesa de Cefaleias), da APED (Associação Portuguesa para o estudo da Dor) e da SPDOF (Sociedade Portuguesa de Disfunção Temporomandibular, Dor Orofacial e Sono).
