Dactilite na artrite psoriática
| Resumo: | Introduction: Dactylitis is a a hallmark clinical feature in patients with spondyarthropaties (EA) more frequent in psoriatic arthritis (AP), yet there are a few studies that characterize and correlate it with other variables. Objective: To describe clinical, laboratory and therapeutics characteristics of a group of patients with AP and dactylitis.Materials and Methods: Descriptive study of a group of patients with AP according to the CASPAR criteria and dactylitis followed in CHLN.Results: 22 patients with AP and dactylitis were included in the study. Current age was 39.1 ± 13,3 years, 59% male gender and the most frequent pattern was symmetric polyarthritis. Most patients were negative for HLA B27 (59.1%), rheumatoid factor (77.3%) and anti-CCP (63.6%). An extra-articular manifestation was present in all patients, namely enthesitis (40.9%). Dactylitis affected feet only in 45.5% of cases being the affected finger’s number of 3.35 ± 3.42. LDI’s basic mean value was 29.1 ± 2.65. 91% of patients were medicated with AINEs, 72.7% with methotrexate. 31.8% received systemic corticotherapy. 27.3% were receiving biologic therapies. Last disease activity evaluation revealed mean values of VS and PCR of 23 ± 25 mm/hr and 0.8 ± 1 mg/dL, respectively. HAQ mean value was 0.5 ± 0.58, patient global activity VAS 35.6 ± 26, DAS28 score for tender joint was 1.47 ± 2.12 and for swollen joint was 1.41 ± 1.87. According to the mean values achieved, the group of patients wasn’t in MDA.Conclusion: Dactylitis most often affected the feet, being rare more than four fingers affected simultaneously. The group of patients with AP and dactylitis presented as clinical predictive factors for progressive disease polyarticular involvement and evidence of physical disability. In mean, they didn’t fulfilled minimal disease activity (MDA) criteria. |
