Transplantação de timo : uma perspectiva literária dos últimos 10 anos

Resumo:The thymus is located at the anterior mediastinum. Histologically is divided in cortex and medulla where positive and negative selection of thymocyte take place, respectively. Embryologically derives from the third pharyngeal pouch’s three layers, being TEC an essencial part of the thymic stroma due to thymopoiesis. Thymopoiesis encloses T cells’ migration, proliferation, differentiation and selection. Thymus transplantation is a very promising but still experimental treatment for athymic pediatric patients’ secondary to complete DGS (typical or atypical) or FOXN1 deficiency. This procedure is able to reconstitute this patients’ immune system and extend their life. Donor and recipients are widely screened before going under transplantation and, in this specific case, HLA and ABO matching is not required. Thymus tissue is properly processed, cultured and transplanted into the quadriceps muscles. About 3 months after surgery biopsies may already show thymopoiesis and circulating naïve T cells, as well as immunoglobulin and antibodies synthesis. Survival rate is higher than 70% with a mean follow-up time of about 4,7 years. Most common complications are infection, predominantly bacterial, autoimmune diseases and particularly thyroid desease.
