Enterobacteriaceae produtoras de carbapenemases : um novo desafio no controlo da infecção hospitalar

Resumo:Carbapenems were first used in clinical practice in 1985 and since then they are considered to be the gold standard therapy for infections caused by extended spectrum beta-lactamases. Nevertheless, many bacteria, including Enterobacteriaceae, started becoming resistant to carbapenems through the synthesis of carbapenemases, thus ruling out all the available antibiotic therapies and making these infections become potentially fatal and capable of frustrating every attempt of treatment. The growing incidence of this kind of infections and the high mortality rates it generates justify the urgency in implementing control and prevention measures to avoid infections caused by carbapenem-resistant bacteria. Therefore, carbapenem resistance in general and carbapenemases in particular are faced as a public health issue of growing relevance worldwide. This paper aims to review recent literature on carbapenemases in Enterobacteriaceae, mainly about their classification, their epidemiology and the risk factors to their appearance, as well as possible control and prevention strategies.
