Polissensibilização alimentar : um caso clínico

Resumo:The authors describe a clinical case of a 33 years old male, without atopic symptoms until he was 16 years old, when he developed allergic rhinitis. Since he was 18 years old, he described multiple episodes of generalized urticaria, sometimes accompanied with dyspnea and facial angioedema, sometimes needing treatment in the Emergency Room. These symptoms were triggered by numerous types of food, but it was only possible to identify several reproducible episodes with peanut. He also describes an oropharyngeal allergy syndrome with the consumption of grape, tomato and banana. After the diagnostic tests (skin prick test, prick-prick skin test and specific IgE), the presence of polysensitization to many types of food of different families was identified, with great predominance of Rosaceae fruits, which was probably explained by a syndrome of sensitization to non-specific lipid transfer proteins (nsLTPs). Several clinical characteristics support this hypothesis, like the severe and systemic symptoms and the tolerance to the ingestion of fruits without the skin; while others are less typical, like the concomitant pollinosis to grass, tolerance to the ingestion of some cooked/processed types of food and the local symptoms. We describe the presence of nsLTPs in multiple types of food involved in this case, pointing out some peculiar aspects. Lastly, we mention the possible therapeutic approaches, proposing this patient as a candidate to sublingual immunotherapy with Pru p 3.
