Tratamento endoscópico endonasal de tumores nasossinusais malignos : resseção e reconstrução

Resumo:The development of the endoscopic techniques enabled the progress in the surgical treatment field and therefore, allowed a new panorama besides the conventional sinusal endoscopic surgery. In the past years, the endonasal endoscopic technique has begun to be implemented in the treatment of malignant sinonasal tumors. Several published studies on patients submitted to endonasal endoscopic tumor resection presented identical results compared with those obtained through the traditional transcranial via, but with less morbidity. There is some controversy about its enforceability, which, among other criticisms, rests on the ability to manage the complications arising from the procedure through cranial reconstruction techniques with nasoseptal and pericranial flaps, as the overflow of cerebrospinal fluid. The surgical training is fundamental in the development of the abilities and efficiency of the proceedings, in order to obtain the best oncological results and the responsiveness to the complications that might be brought. There is, however, a long way in this area to go before, even though the promising results up to then regarding the enforceability of the endoscopic endonasal surgery for the resection of sinonasal tumors and reconstruction base skull.
