Amaurose e alterações neuropsiquiátricas como apresentação de neurossífilis

Resumo:Neurosyphilis consists in the central nervous system involvement by Treponema pallidum subspecies pallidum. This involvement can occur at any stage of syphilis natural history, and it includes both symptomatic and asymptomatic presentations. This is a rare condition, with multiple clinical presentations, a challenging diagnosis, and variability in response to the standard treatment, and it should always be considered in the differential diagnosis of neurologic and psychiatric symptomatology. It is described a clinical case of a 69-year old male patient brought to the emergency room with long-term progressing neuropsychiatric ymptomatology, with sudden onset amaurosis fugax of the right eye. The Venereal Disease Research Laboratory analysis in the cerebrospinal fluid was positive, thus establishing the diagnosis of neurosyphilis. The purpose of this paper is to provide a global and current clinical perspective about neurosyphilis, reviewing untreated syphilis natural history, different clinical presentations, diagnostic methods, and treatment and follow-up recommendations.
