Terapêutica da amigdalite aguda em idade pediátrica : estudo transversal sobre antibioterapia nos cuidados de saúde primários e urgência hospitalar em 2012
| Resumo: | Acute tonsillitis is a very common ilness among children. Clinical findings and epidemiological data alone are not useful in the determination of the etiological agent. The Rapid Antigen Detection Test (RADT) allows for the diagnosis to be made in a few minutes. A recent Portuguese guideline (NOC 020/2012) – which is still in public discussion – establishes its use in pediatric ages, in order to guide the treatment. The first line treatment for bacterial tonsillitis is oral amoxicillin. In this study we’ve intended to evaluate the prescription of antibiotics and the relation between that and the use of RADT in pediatric patients with the diagnosis of acute tonsillitis in the pediatric emergency room of Santa Maria Hospital (HSM – a tertiary hospital) and 5 primary care units (Unidades de Saúde Familiar Carnide Quer, Gerações, Parque and Unidades de Cuidados de Saúde Personalizados Alvalade and Benfica). We’ve included in the study 746 patients from HSM and 318 patients from primary care. In HSM and antibiotic was prescribed in 64,34% of the cases vs. 80,82% in the primary care units. Amoxicillin was the first choice antibiotic in HSM (53,13%), whereas in primary care it was the combination of amoxicillin and clavulanic acid (45,14%). The real cost per patient was higher in HSM (5,11€ vs. 4,23€). Had only been prescribed amoxicillin, the cost would have been significantly lower (4,06€ and 3,09€). If the guidelines had always been followed, it would have been 6,53€. |
