Revisão teórica do diagnóstico e tratamento de aspergilose broncopulmonar alérgica : estudo casuístico do internamento do Serviço de Pneumologia entre Janeiro de 2010 e Setembro de 2014

Resumo:Allergic bronchopulmonary aspergillosis (ABPA) is an immunological pulmonar disorder caused by hypersensitivity to Aspergillus fumigatus, manifesting with poorly controlled asthma, recurrent pulmonar infiltrates and bronchiectasis. The importance of controlling symptoms and preventing irreversible pulmonar lesions explains the necessity of recognizing and diagnosing ABPA. Based on a literature review, the aim of this study was to analyse the work-up and treatment process of the patients hospitalized in the Pneumology Department of the Santa Maria Hospital, from January 2010 to September 2014. Administrative databases were provided and therefore analysed. During the study period, 2180 hospitalisations occured in the Pneumology Department. Asthma exacerbation was the main or secondary diagnosis of 116 occurrences; ABPA was the main or secondary diagnosis of 7 hospitalisations. This corresponds to a total of 4 patients. Due to the small number of clinical cases, a statistical analysis wasn’t possible. From the study of the 4 clinical records, not only the clinical finds but also the work-up process were in agreement with the most recent literature. It was hypothesized that the small patient sample could be related to the absence of a database from the Ambulatory. From the literature review of this disorder, the author concluded that new diagnosing tests are currently being investigated and promising new therapies are emerging concerning the study of the Th2 pathway.
