Shoulder pain in overhead athletes : a systematic review of orthopaedic causes

Resumo:This study was aimed to compile the most important diagnosis of shoulder pain in overhead athletes. In overhead sports as the arm is abducted or rotated and the subacromial space width changes and the cuff becomes increasingly compressed. The causes of shoulder pain and dysfunction in the overhead athlete are controversial with several proposed theories, including subacromial impingement, occult instability, tendon tension overload, and internal impingement. It is understood that shoulder injuries causing pain in overhead athletes are usually, despite other possible diagnosis, the result of a continuous process involving several stages of development where different diagnosis can be made. These theories process a three to four groups of pathologies that goes from rotator cuff pahtology, labral pathology, shoulder instability. Starting from the physiopathology of the shoulder impingement we can now develop a reasoning that includes two major groups of injuries in overhead athlete context: tendinopathies with tendinitis, tendon tears and subacromial conflit; multidirectional and anterior/posterior instabilities as well as labrum lesions.
