Copenhagen Burnout Inventory and Zarit Burden interview in formal caregivers : exploratory and confirmatory factorial analysis

Resumo:The aim of the present study was to examine the reliability and construct validity of theoretically determined Three-dimensionality of the Copenhagen Burnout Inventory (CBI; Borritz et al., 2006; Kristensen, Borritz, Villadsen, & Christensen, 2005), and exploring the applicability as well as the factorial structure of the Zarit Burden Interview (ZBI; Sequeira, 2010; Zarit, Reever, & Bach-Peterson, 1980), in a group of formal caregivers of Portuguese population. Method: The factor analysis was carried out using Exploratory Factorial Analysis (EFA) and Confirmatory Factorial Analysis (CFA), to psychometrically evaluate CBI and ZBI. The EFA and CFA are widely used in measurement applications for construction validation in an instrument, in a population different from the one in which the instrument was developed. The present study population consisted of 183 formal caregivers who responded the CBI and out of which 123 have answered also to the ZBI. The Data was collected on the basis of combination of 7 studies carried out at different times across the country, through self-administered questionnaires of the CBI and the ZBI, on caregivers who were working either in health care centers, hospitals, established institutions or providing home-based care to the sick/elderly. Results: The EFA showed a good internal consistency of theoretically defined Three-dimensional Model of the CBI. Furthermore, the exclusion of two items (2 and 19), belonging to Work-related Burnout, presented better results. The CFA verified the model fit indices of the Three-dimensional Model without items 2 and 19. Hereafter, PersonalBurnout consisted of 6 items, Work-related Burnout of 5 items and Patient-related Burnout comprised of 6 items. With regard to the applicability of the ZBI in formal caregivers, the results showed a good internal consistency of the scale and identified a Six-factor Model of the Burden for formal caregivers. The results of CFA confirmed a multidimensionality of the scale when compared with various statistically driven Models of informal caregivers, henceforth reinforcing the possibility of six factors extracted in EFA. The factors were identified as: Subordination of the Person in Role, Perception of Exclusivity, Emotional Impact, General Dissatisfaction, Perception of Auto-efficacy, and Emotional Exhaustion. Conclusion: The CBI has adequate validity and a high internal consistency as an instrument to evaluate the Burnout in formal caregivers. The ZBI, having indicated a good reliability, can be an adequate instrument to evaluate the presence of the Burden in formal caregivers.
